STATIN USE INCREASE DIABETES RISK

Long-term statin use is associated with an increased risk of type 2 diabetes of approximately 30% in individuals at high risk of the disease, even after taking into account known risk factors and potential confounders, say US researchers.
They looked at the development of diabetes among statin users in the Diabetes Prevention Program (DPP), which included more than 3200 participants.
Over 10 years, statin use was linked to a 36% increased risk of being diagnosed with type 2 diabetes, falling to 27% after taking into account baseline risk factors and clinical criteria used to determine the need for statins.
The findings are consistent with previous studies suggesting that statin use substantially increases the risk of type 2 diabetes.
The new study was published online October 23 in BMJ Open Diabetes Research & Care by lead author Jill P Crandall, MD, department of medicine and diabetes research center, Albert Einstein College of Medicine, New York, New York, and colleagues.
As previously reported by Medscape Medical News, a study of more than 8700 Finnish men aged 45 to 73 years showed that over 6 years of statins therapy were linked to a 46% increased risk of type 2 diabetes — more than double prior estimates.
This was followed by recent data from the Australian Longitudinal Study on Women's Health, which indicated that, among almost 8400 women aged 76 to 82 years, the risk of new-onset diabetes ranged from 17% with the lowest statin doses to 51% with the highest doses.
Despite accumulating evidence, the current researchers still maintain that the overall healthcare advice remains unchanged — the benefits of statins outweigh the risks.
"For individual patients, a potential modest increase in diabetes risk clearly needs to be balanced against the consistent and highly significant reductions in myocardial infarction, stroke, and cardiovascular death associated with statin treatment," they state.
They add: "Nonetheless, glucose status should be monitored and healthy lifestyle behaviors reinforced in high-risk patients who are prescribed statins for cardiovascular disease prophylaxis."

First Look at Diabetes Risk From Statins in High-Risk Patients

The researchers point out that the diabetogenic effect of statins has previously been studied in individuals typically at relatively low risk of diabetes and that the incidence of disease was based on self-report, rather than being the primary outcome.
They therefore set out to examine the issue in DPP participants, which included 3234 US adults randomized to intensive lifestyle intervention, metformin, or placebo.
After a mean follow-up of 3.2 years, participants were invited to take part in the DPP outcomes study, which included quarterly lifestyle sessions alongside open-label metformin for those originally randomized to the drug and two additional lifestyle programs per year for those originally randomized to the lifestyle intervention.
Approximately 50% of DPP participants were from ethnic groups and 20% were aged ≥ 60 years. To be included, they had to be aged ≥ 25 years and have a body mass index ≥ 24 kg/m2. They also had to have a fasting plasma glucose (FPG) of 95–125 mg/dL and impaired glucose tolerance, placing them at high risk of type 2 diabetes.
The primary end point was diabetes diagnosis using an annual 75-g oral glucose tolerance test or semiannual FPG level with repeat confirmation testing. Lipids and blood pressure were assessed annually and statin use was based on self-report.
From a baseline of approximately 4%, statin use progressively increased at the 10-year follow-up to reach 35% in the placebo group, 37% in the metformin group, and 33% in the lifestyle intervention group (P = .36 between groups).
Simvastatin was the most commonly used statin, taken by 40% of participants, followed by atorvastatin by 37% and, much less frequently, lovastatin and pravastatin, by 9% and 8%, respectively.
Statins users were typically older and more likely to be men than nonusers but did not differ by ethnicity. Compared with nonusers, they also had modestly higher baseline FPG and HbA1c, higher baseline low-density lipoprotein (LDL) cholesterol and triglycerides, and were more likely to have a history of cardiovascular disease and hypertension.
Taking into account age, sex, and ethnicity, researchers found that statin use was associated with a significantly increased risk of developing diabetes, at a hazard ratio (HR) of 1.36 for all three groups combined.
Further adjustment for baseline diabetes risk factors, including family history of diabetes and FPG, reduced the HR to 1.35, and additional adjustment for statin treatment confounders, such as blood pressure, cholesterol levels, baseline cardiovascular risk factors, and socioeconomic status, lowered the HR to 1.27.
Diabetes risk did not differ depending on statin potency or magnitude of LDL-cholesterol reduction, although longer statin use was significantly associated with greater diabetes risk (HR per visit with statin use, 1.06; P=.007).

Mechanisms Underlying Effect "Poorly Understood"

Discussing the findings, the researchers say that "it has been suggested that statins may 'uncover' diabetes in individuals at high risk, which on a population basis, could result in modest increase in diabetes risk."
They point out, however, that "variation in baseline diabetes risk factors failed to explain the further risk associated with statin therapy in our cohort, and the HR estimate was greatest among our lifestyle participants, who experienced the largest study-related reductions in diabetes risk."
The mechanisms underlying any diabetogenic effect of statins are "poorly understood," they add.
Although several studies have looked at changes in insulin sensitivity during statin use, "we saw no evidence of an effect of statins to modify insulin resistance, assessed as fasting insulin concentrations," they write.
And although statins have been reported to reduce pancreatic beta-cell insulin secretion in vitro, "the relevance to insulin secretion in vivo is not known," they state.
Taken together, evidence from studies published so far points to "an acceleration of typical glycemic deterioration, rather than a unique or statin-specific mechanism," they conclude.

Excess alcohol is linked to a greater risk of high blood pressure, high levels of blood fats, and heart failure. In addition, the extra calories can lead to weight gain, a threat to heart health.

If you drink, stick to no more than two drinks per day for men, and no more than one a day for women. (One drink means a 12-ounce beer or 4-ounce glass of wine).


Overeating

Being overweight is a major risk factor for heart disease, and 72% of men and 64% of women in the U.S are overweight or obese.

Try to eat less, avoid oversize portions, and replace sugary drinks with water.

Dr. Reynolds and Dr. Hochman also suggest cutting portion sizes for high-calorie carbohydrates (think refined pastas and breads) and watching out for foods labeled “low-fat,” which are often high in calories.

"Don't assume you're not at risk," says Dr. Ostfeld.

Eating red meat

It’s best to think of red meat as an occasional treat rather than the foundation of a daily diet. Red meat is high in saturated fat, and there’s also evidence that processed meat, such as bacon and hot dogs, increases your risk of cardiovascular disease and colorectal cancer. Ideally, less than 10% of your diet should come from animals and animal products, Dr. Ostfeld advises.

Can’t part with the beef? Choose a lean cut of red meat and limit your intake. “People have to know that if you want a steak a few times a month, it’s OK,” Dr. Hochman says. “It’s what you’re eating three times a day that’s the issue. Be in it for the long haul. Eat a balanced diet.

Let's be honest: Taking pills is a pain. There can be side effects. And it's easy to forget your meds, especially if you feel fine.

"High blood pressure is called the silent killer because you don't feel it," Dr. Ostfeld says. "Saying you feel fine is not a justification for stopping these pills."

There are 30 types of high blood pressure medications, so there are choices if one isn't working, Dr. Hochman says. "If one medication doesn't work, we can try something else."


Avoiding fruits and vegetables

"The most heart-healthy diet is a plant-based diet," Dr. Ostfeld says. That means loading up on fruits and vegetables, nuts, whole grains, low-fat dairy, and protein, and keeping junk food to a minimum. In fact, new federal dietary guidelines recommend that half of each meal should be composed of fruits and vegetables.

Research has found that people who eat more than five servings of fruits and vegetables a day had about 20% lower risk of heart disease and stroke than people who ate less than three servings per day.


Ignoring physical symptoms

If you used to walk up three flights of stairs without a problem, but suddenly you're short of breath after one flight or have chest pressure, it's time to call your doctor—now. Never assume it's because you're out of shape.

Doctors say "time is muscle," meaning the quicker you get treatment for possible trouble, the less likely you are to have permanent damage to your heart muscle.

"It's better for it to be much ado about nothing than sitting on a heart attack for six hours," which is not uncommon, Dr. Ostfeld says

“Steer clear of packaged junk food, read the labels for sodium content, and stick to the outer portions of the supermarket, which is where the fruits, vegetables, and (unsalted) nuts are,” Dr. Ostfeld says.

Most of us should keep sodium intake below 2,300 milligrams a day. If you have high blood pressure or are over 50, cut back to 1,500 milligrams.

New medication guidelines published Dec. 18 in the Journal of the American Medical Association (JAMA) suggest that not everyone with high blood pressure needs to be on drugs for it.
Here's the bottom line: If you are 60 or older and the first number of your blood pressure is less than 150, you don't necessarily need to be on medication for it.
Keep in mind that this choice is up to your doctor — blood pressure is just one risk factor for heart disease, and people using medication to maintain a number in the 140s without serious side effects should not suddenly stop and let that number drift up.
The new guideline will decrease the number of people medicated for their high blood pressure, but not everyone's happy about it.

But not everyone gets their blood pressure low enough with just lifestyle changes, and if that first number climbs above 140 — a condition affecting about two-thirds of people 60 and older — you officially have high blood pressure, or hypertension.
Until now, that's when doctors suggest medication to help lower BP. About 50 million Americans are on blood pressure drugs of all different kinds, including diuretics ("water pills"), beta blockers (which slow your heartbeat), and ACE inhibitors (which help stop blood vessels from narrowing).
Why change the guidelines?
"Over the last ten years, doctors have had the notion that the lower blood pressure is, the better," study researcher Paul A. James, of the University of Iowa, told Business Insider.
The problem? While there's no question that medication is crucial for people at high risk of heart attack and stroke, the researchers concluded that there's not enough evidence that driving that number all the way to 140 — rather than simply to 150 — provides much additional benefit.
The panel arrived at the new guidelines after reviewing previous studies that looked at how patients fared on a variety of approaches to lowering BP — different drugs and exercise regimens, for example.
Blood pressure drugs, like any medications, have some side effects (such as dizziness), which can be especially serious in older adults. So, the new guidelines may come as a relief for some.
Still, says James, "I'm absolutely sure there will be controversy."
Not everyone's happy
James is right. Some people are wary that the new guidelines aren't officially endorsed by the National Heart, Lung, and Blood Institute (NHLBI).
The NHLBI initially assembled the panel but has since stopped being involved in making guidelines, so in the end they weren't certified by any official organization. Eric D. Peterson of the Duke Clinical Research Institute told Business Insider that this lack of official endorsement may leave some practicing doctors scratching their heads about how to proceed.
Most experts point out that more research needs to be done. "We have very limited data to tell us what the right thresholds are," said Peterson.
While he agrees that no study says 140 is the magic BP number for beginning treatment in older adults, he notes that one major study showed a notable benefit when older patients brought their number from the mid-150s to the mid-140s — so 150 isn't necessarily right either.
Though Peterson agrees that too-aggressive treatment to lower a patient's blood pressure down to a specific number can harm overall health, he compared the target numbers to speed limits: If you tell people the maximum speed is 65, they may drive at 75. Similarly, leaving the target at 140 may mean more Americans actually bring their number below 150.
"There is always some slippage between targets set for clinicians and [those] actually achieved in routine practice," he writes in an editorial in the same issue of JAMA.
